二重盲検試験で有効性が示されているのは、Cyclophosphamide（2000mg内服）+プレドニゾロン（60mg/日）を月に1回6クールで施行した論文（Neurology 2007）のみです。
一方で、最近ではRituximab（抗CD20抗体）の有効性が小数例で報告されています。
現実的には、以下のような治療オプションになると思います。

1.Cyclophosphamide：600mg/m^2を1回/月、6クール
2.Cyclophosphamide（2000mg内服）+プレドニゾロン（60mg/日）を月に1回6クール
3.Rituximab（抗CD20抗体）
4.免疫グロブリン大量静注療法：有効性は高くありません
5.FludarabineやCladribine

血漿交換療法やIFNαの有効性は示されていません

Pramipexole（ビ・シフロール）はBH4 deficiency欠損症の一つである6‐ピルボイル‐テトラヒドロプテリンシンターゼ欠損（PTPSD）のパーキンソン症状の改善効果があった

成長してからフェニルケトン尿症と診断され重度の行動障害を伴う患者に対するフェニルアラニン制限食は，患者の行動評価スケールでは効果が見られなかったが介護者の評価では多少の効果が示されている．

1.絶食、補液、酸素投与
2.抗生物質
A.市中肺炎

ユナシン（Ampicillin/sulbactam）6g/日
ロセフィン（CTRX）1g/日＋AZM 500mg/日
ミノマイシン100-200mg/日：βラクタムアレルギーの場合

B.誤嚥性肺炎
以前使用されていたダラシンは偽膜性腸炎の発症率が高いため、最近は以下のような抗生剤が選択されることが多いと思います。

ユナシン（Ampicillin/sulbactam）6g/日
タゾシン（Piperacillin/tazobactam; PIP-TZ）2.5-5.0g/日
ロセフィン（ceftriaxone）1g/日

3.ネブライザー

生食3ml+べネトリン0.2-0.5ml+ビソルボン1A　など

4.誤嚥性肺炎の予防

口腔ケア
ACE阻害薬（タナトリルが最もよい）内服
プレタールの少量投与も効果があるようです
胃瘻造設
